To catalyze and instill a sustainable, community-based commitment to care coordination excellence for America’s military connected members and their families. We do so by providing innovative practice, actionable strategies, technology, resources, and expertise to communities, organizations, and individuals who are willing to improve upon their efforts to serve the needs of the nation’s all-volunteer armed forces, veterans, and their families.
The first community-based, coordinated network of services and care for our nation’s military-connected members and their families
AmericaServes utilizes Unite US technology to streamline referrals between the coordination center and participating providers by identifying appropriate services, capacity, and eligibility for their clients among in-network providers who are able to meet human and social service providers spanning 20 service domains. Community networks are typically comprised of 60-70, local human service organizations from the public and private sectors. The aim is for military connected clients to receive efficient, timely, and comprehensive access to the resources they need, where they are.
